This is an example of concept art that is created in multiple stages. First I sculpted the character in Zbrush. It was then rendered in realistic lighting and the images were brought into Photoshop. I painted the color in Photoshop, applying layers of base color and extra skin details such as spots, tones and other flesh-like color. I also composited hair taken from stock footage of an older Asian man as well as smoke from a photograph.

This is a fast way to achieve a realistic look that also allows for much experimentation and adjustments based on client notes.
